Description
Corinfar is a prescription medication used to treat : certain types of chest pain (known medically as angina); high blood pressure (known medically as hypertension).
100 tablets 10mg
Pharmacological action:
Corinfarum - drug group of calcium channel blockers, which has significant anti-anginal and antihypertensive effect. The preparation Corinfarum enters the active substance nifedipine - synthetic derivative of dihydropyridine. Nifedipine is the first generation of selective calcium channel blockers, dihydropyridine derivatives. The mechanism of action of the drug is related to its ability to slow down the penetration of calcium ions into the cells of the smooth muscle layer of blood vessels and the heart through the slow calcium L-type channels. By decreasing the concentration of calcium ions in myocardial cells and vascular smooth muscle layer occurs decrease the contractile activity of the cells of the vascular wall, increased peripheral and coronary vessels. The drug has no effect on the venous vascular wall without preload alters not cause orthostatic hypotension. In various diseases that are accompanied by a violation of the transmembrane current of calcium ions, including hypertension, drug Corinfarum helps normalize the passage of calcium ions through the slow calcium channels.
Effectiveness of the drug in the treatment of patients with coronary heart disease is also based on its ability to reduce myocardial oxygen demand by reducing the mechanical work of the heart. Furthermore, due to the expansion of peripheral vascular resistance and decreases peripheral afterload on the heart. Expansion of coronary microcirculation and helps to improve myocardial oxygen supply.
The hypotensive effect of the drug is carried by peripheral vascular relaxation and reduce total peripheral resistance with no changes in heart rate. For the drug is characterized by a mild diuretic effect with increased sodium excretion, so that there is a reduction in blood volume and total peripheral vascular resistance.
In addition atianginalnogo and hypotensive effect of the drug has also expressed organo effect, in particular for the preparation and nephroprotective characteristic cardioprotective effect. Cardioprotective effect of the drug caused a simultaneous decrease in blood pressure and total peripheral resistance, which leads to decrease afterload and myocardial oxygen demand. In applying the drug, patients have improvement in diastolic function. Renal protection effect of the drug is associated with the elimination of renal vascular spasm, improving renal blood flow and an increase in glomerular filtration rate.
Nifedipine can inhibit platelet aggregation due to decreased synthesis of prostaglandins proagregantnyh.
In clinical studies, it has been found property nifedipine slow the development of atherosclerotic changes in vessel walls.
The ingestion of nifedipine are well absorbed in the gastro-intestinal tract. After sublingual therapeutic drug effect occurs within 5-15 minutes after oral application - for 10-30 minutes. Maximum therapeutic effect is observed after 30-90 minutes after drug administration, the effect of the drug is maintained for 4-6 hours. With a single application of 2 tablets of Corinfarum therapeutic concentration of drug in plasma is reached within 30 minutes, the peak concentration of the active substance in blood plasma was after 1.6 hours, after which the drug concentration decreases gradually.
In applying the drug Corinfarum Retard nifedipine therapeutic concentration in the blood plasma observed 60 minutes after oral administration, peak plasma concentrations of nifedipine noted after 2.4 hours, the equilibrium concentration of the drug resistant maintained for 6 hours. Therapeutic concentrations of nifedipine in plasma observed after 10 hours after taking the drug Corinfarum Retard. After 24 hours after a single drug application Corinfarum Retard plasma nifedipine concentrations of about 2 ng / ml.
In applying the drug Corinfarum DNA maximum concentration in plasma is reached within 4-6 hours, the therapeutic effect lasts for 24 hours.
The bioavailability of the preparation is from 50 to 70% in different patients, due to the pronounced effect of first passage through the liver, which causes the variation of the concentration of active ingredient in some patients. For preparation characterized by a high degree of association with plasma proteins (95%).
Drug metabolism occurs in the liver, presumably by oxidation and hydrolysation. As a result of the metabolism of nifedipine produced pharmacologically inactive compounds.
Withdrawal of the drug primarily by the kidneys in the form of pharmacologically inactive metabolites, less than 1% of the drug is excreted unchanged by the kidneys. About 15-30% of the metabolites excreted in the feces.
The half-life of the drug reaches 5-11 hours. Accumulation of the drug in the body is not observed even after prolonged use of the drug at therapeutic doses.
In patients with impaired liver function observed half-life extension. In most cases, patients with impaired liver function requires correction dose Corinfarum.
Preparation is well into the various organs and body fluids, so nifedipine penetrates the blood-brain barrier and gematoplatsentrany. Marked by the excretion of the drug in breast milk.
Due to the high degree of association with plasma proteins nifedipine practically displayed during hemodialysis.
Indications for use:
The drug is used to treat patients suffering from various diseases of the cardiovascular system, including:
- High blood pressure, including essential hypertension;
- Ischemic heart disease, including angina, variant angina and Prinzmetal angina.
How to use:
Corinfarum:
Adults usually appoint 1-2 tablets of the drug 2-3 times a day at regular intervals. The tablets are swallowed whole without chewing or crushing, drinking plenty of water. It is recommended to take the drug after meals. The duration of treatment determined by the attending physician depending on the nature of the disease.

The maximum daily dose of nifedipine - 80mg, single - 40 mg.
Patients suffering from severe hepatic insufficiency, a correction dose.
Side effects:
The drug is generally well tolerated, however, in some patients, especially at the beginning of therapy with Corinfarum may develop these side effects:
On the part of the gastrointestinal tract and liver: nausea, vomiting, discomfort in the stomach, the violation of a chair, reversible gingival hyperplasia. With long-term use of the drug in a few cases of adverse effects noted by the liver, including violation of the outflow of bile, hepatitis, elevated liver enzymes, transient increase in serum transaminases.
Cardio-vascular system: the flow of blood to the head and upper body, which is accompanied by hyperemia of the sites and the sensation of heat, reflex tachycardia, a sharp drop in blood pressure, increasing angina attacks, swelling of the extremities. In rare cases, patients had a myocardial infarction.
Hematopoietic system: anemia, thrombocytopenic purpura, thrombocytopenia, leukopenia.
Of the central and peripheral nervous system: headache, dizziness, paresthesia, irritability, drowsiness, sleep and wakefulness, fatigue, tremor.
Allergic reactions: skin rash, itching, rash, exfoliative dermatitis.
Other: were marked by sporadic cases of hyperglycemia in patients with diabetes. In older men taking the drug Corinfarum, in a few cases, it was noted the development of gynecomastia, which took place after the withdrawal of the drug. In addition, when using the drug may develop visual impairment and myalgias.
If rapid elimination of the drug may develop hypertension and myocardial ischemia, so stop taking the drug recommended by gradually reducing the dose.
